Repeat Prescriptions

Patients on regular medication will be issued with a repeat order form by their doctor. This is printed on the counterfoil at the right of the computer-printed prescription. To request a repeat prescription please use one of the following methods:

• Use our On-Line Services from this website, or
• Tick the items you require on the prescription slip and deliver to the surgery (drop through our letterbox when we are closed), or 
• Post to the Surgery, or 
• Fax your request to the Surgery on 01794 514224.

We are unable to take requests by telephone for medico-legal reasons.
