New Patients

We welcome new patients to the Abbey Mead Surgery.

When you come to register please bring your medical card or details of your previous address and Doctor's surgery.

You will be welcomed by one of our Reception Staff who will tell you about the practice, and ask you to complete a "New Patient Health Questionnaire" and a Registration Form for each person registering with the Practice.

You will then be asked to make an appointment at your convenience to see your new doctor or one of the practice nurses for a brief health check. This serves to advise us of your health needs as soon as you join the surgery, whilst waiting for your previous records to reach us.
